About The Company:

Care Path Connections is looking to hire some good-hearted individuals who care about the senior community and who want to have a hand in keeping our seniors well cared for. We are a locally owned and operated company out of Tacoma, WA who prides ourselves on putting seniors first, leading with compassion and being effective care plan navigators for our clients. We are up and coming with 10+ years in the placement advisory industry. Our goal is to build a team of confident and capable Advisors excited about helping seniors all throughout WA.

About The Position:

As a Placement Advisor with Care Path Connections, you will be responsible for helping seniors and their families determine the best 24hr care options for their individual circumstances and needs. You will be the bridge between patient's and the quality long term care they need. This position offers a flexible schedule, training and support, marketing supplies, as well as opportunity for growth. It is ideal for someone who wishes to make extra or supplemental income at first but has goals to build a significant yearly income with time and effort. This is a commission only position with a 50/50 split. Advisors take 50% of each successful referral fee paid. The referral fees range from $1,000-$10,000 with the average being $5,000 per placement.

Primary Responsibilities:

Screen 24hr care options and establish referral contracts.

Market services to the local area to generate referral sources.

Keep consistent contact with established referral sources.

Complete intake interviews with referred seniors and their families to determine, care needs, important wants, and

circumstances for placement.

Determine ability to meet client's needs.

Determine care options and explain the options to client's and their families.

Schedule tours/visits from clients and/or their families at the appropriate care facilities.

Accompany families/clients to appropriate community tours when requested.

Facilitate a smooth and successful transition for seniors from a home or healthcare setting into a community, adult

family home, or other relevant care setting.

Keep complete client documents after each client contact in an organized and easy to access file.

Follow up with placed seniors/families to ensure quality services.

Always and foremost be an advocate for the seniors and the families that we assist at every step of placement.

Have strong people skills with ability to effectively create connections.

Can function independently, have flexibility, honor, and the ability to work effectively with community partners.

Must demonstrate a positive, can-do attitude, highly disciplined and determined to meet goals while working

independently most of the time.

Is open to suggestions, a problem solver, and able to find solutions and or ask for help from management when needed.

Ability to maintain consistency and determination.

Ability to think and adapt to an out of the box work environment.

Have a respectful, neat, professional, courteous manner and appearance.

Qualifications/Educational Preferences:

Experience working in home care, senior housing, senior health, hospice, healthcare admissions, healthcare

discharge planning, social work, psychology, sales or similar environments.

Ability to work independently with minimum supervision, problem solving, conflict management in a professional way.

Possess excellent communication skills.

Basic computer proficiency.

Strong verbal and written communication skills.

Reliable personal transportation.

Persistent and consistent.

Schedule: Part-time/Full-time

Training will be held in Federal Way, WA and will require availability for a determined set schedule the first week. After the first week you will be required to be available two days a week for 3-5 hours for in the field experience. After training this is a 100% flexible schedule position, you will be able to make your own work schedule based on your availability and the business hours of your assigned area. Keep in mind consistency and determination in efforts will be the deciding factors in your success.

Pay: $10,000.00 - $75,000.00 per year, this in an uncapped commission position. What you make will be up to your consistency, determination and effort.

Work Location:

Multiple locations, we are hiring advisors for the Tacoma, Lacey/Olympia, Issaquah and Seattle areas. You will work in the area closest to where you live with seldom need to go elsewhere. We do our best to keep our advisors local to their hometown.
