Elder care provider needed

In search of an elder care provider, male or female, 5-7 hours per day (either shift: 10 AM-3 PM, or 3-10 PM), for an 86-year-old man with mobility problems and mild dementia. He needs assistance with walking (with walker), being lifted into wheel chair or bed, hygiene, and occasional meal prep. Activities could include taking him outdoors in wheel chair, conversation, help with bathing/shaving/bathroom.
