Embrace Journey LLC is hiring a Case Worker to join our growing team! We are in search of an individual who has the ability to provide the best possible community-based support for aging adults, people with disabilities, and people affected by complex, chronic health conditions.
JOB SUMMARY: As a Case Worker, you will help monitor the quality of care and services as well as ensure our clients have choice and independence. You will advocate with and on behalf of our clients to provide care-coordinated, problem-solving abilities (sometimes on an urgent basis), and ensure our clients maintain their anatomy. In addition, you will manage monthly progress notes, caseload files, and correspondence.

Pay: $27 per hour
Mileage reimbursement
Hours: This is a 12-15-hour a week job, schedule is flexible typically Monday through Friday but may vary depending on customers’ needs.
Knowledge, Skills, and Abilities:
Demonstrated ability to assess client health and functional status, understand information listed on care plans, assess
skills of caregivers, and make clear and concise recommendations.
Demonstrated client advocacy skills and sensitivity to the needs and values of diverse groups.
Ability to work independently, with good judgment and a minimum of supervision.
Ability to plan, prioritize, and coordinate work assignments and/or projects.
Ability to establish and maintain effective working relationships with coworkers, community agencies, and the
public using courtesy, tact, and good judgment.
Ability to defuse difficult situations recognizing the need for sensitivity as well as assertiveness.
Qualifications:
1. Excellent written and verbal communication skills.
2. Proficiency in Google Suites and PDF Files
3. Reliable, Patient, and Self-Motivated
Licenses/Certificates/Special Requirements:
Must have a valid Washington State driver's license, a vehicle available for work-related travel, and appropriate
liability insurance that meets agency policy standards.
Must pass Washington state background check.
No open/closed cases with Adult Protective Services (APS) and/or Child Protect Services (CPS)
