Part -time, 80% FTE, 32 hours per weekDEPARTMENT DESCRIPTION: Harborview’s downtown programs focus on Harborview’s mission to address health equity issues in our community. Our Downtown Programs ensure access to care for those experiencing or at risk for homelessness in our primary care settings, homeless shelters, housing programs and in the community. This is a unique opportunity to be a part of a collaborative approach to client-centered care provision.Health One was launched by the Seattle Fire Department’s Mobile Integrated Health Program in 2019 to reduce low-acuity EMS calls and ED utilization. Dedicated teams of EMT-firefighters and experienced Human Services Department case managers provide outreach and respond to community members with low-acuity concerns, providing on-site evaluation, crisis intervention, referrals, and warm handoffs to community resources. The APP (ARNP or PA-C) will support the Health One team by providing more in-depth assessments, prescribing necessary medications, ordering diagnostic testing, and linking clients with fixed-site medical care in one of Harborview’s primary care clinics.POSITION HIGHLIGHTS:

This is an outreach position with home bases at Hobson Place Clinic in north Beacon Hill and Seattle Fire Department Headquarters in Pioneer Square, however, it will require daily travel throughout Seattle as part of the Health One mobile outreach team.PRIMARY JOB RESPONSIBILITIES:

Provides comprehensive assessments of complex/specialized patients, and when appropriate, their families.

Supports the Health One team providing care in the field and assisting with care coordination for vulnerable members of the community.

Obtains and records medical histories, performs physical examinations, orders and interprets diagnostic tests, establishes diagnosis and follows current standards of practice for prescribing medications and/or establishing treatment protocols. Recognizes own limitations and consults when appropriate.

Recognizes deviations from normal in the physical assessment and works in collaboration with the physician to develop management and treatment plans.

Documents findings, reports and plans of care including needed patient education and appropriate follow up of care in the medical record.

Performs efficiently during emergency situations, using sound judgment and following established protocol.

Participates in the QA and process improvement activities of the program. Shares in the improvement in quality of patient care through participation in committees, implementation of new trends in nursing or medical care, and by contributing suggestions for improvement in organized delivery of care.

Participates in an interdepartmental and inter agency effort to facilitate a safe environment for staff and patient care.

Follows HMC and Seattle Fire Department standards on the use of safety equipment and regulations for personnel (i.e., blood borne pathogens, TB, etc.).

Assists in the training of various levels of health care professionals.

Delegates tasks and duties for the purpose of coordinating effective and efficient health care.

Develops and maintains relationships with staff of both agencies (Harborview Medical Center and Seattle Fire Department) including with clinic staff, clinic manager, clinic medical director, interdisciplinary patient care teams and other hospital staff as well as Health One Case Managers, Fire Fighters and Health One administration, to ensure maximizing positive department and employee relationships.

Acts as a liaison between agencies, assisting with mutually beneficial collaborations for the benefit of patients.

Utilizes appropriate lines of authority to maintain effective communication.

Supports and advocates the mission and objectives of the medical center and the Health One program and assists in interpreting these to patients, visitors and the community.

Demonstrates professional responsibility and leadership through participation in hospital, University, professional, and educational opportunities.REQUIREMENTS:

Nurse Practitioner:

Master’s Degree in nursing from an accredited school of nursing

AND Washington State Advanced Registered Nurse Practitioner License with prescriptive privileges

AND National certification as a Nurse Practitioner

AND ACLS certificationOR Physician Assistant:

Licensure as a Physician’s Assistant in the State of Washington with national certification from a national certifying body (NCCPA).

Applicant must be eligible to receive hospital staff appointments for adjunct staff membership by the Organized Medial Staff of the hospital under Medical Staff By-Laws for hospital-based positions.
