Job Description: Caregiver

Reports to: Adult Family Home Manager

Qualifications:

1. Minimum age of 18 years.

2. Be able to pass criminal background check.

3. Current CPR/First aid card

4. Current state certification without restriction NAC, HCA or NAR that is delegable

The right person must have or be obtaining certification for Nurse Delegation, including Diabetes within 14 days of employment and be willing to be delegated

5. Valid driver's license or Washington State ID, or a means of prompt, reliable alternative transportation with current auto insurance.

6. Have current contact information on file at the AFH, that includes a home telephone number and address.

7. High school graduate or equivalent.

8. Ability to follow instructions organize, and set priorities, and have the ability to work well with other staff and residents.

9. Must be able to safely and successfully perform the assigned tasks and job-related function, with or without reasonable accommodation required by federal, state or local law; and with no pre-existing conditions that would prevent the employee to perform a task.

Responsibilities:

Provide cueing, assistance, and support to multiple residents in all areas of daily living as outlined in the resident's NCP (Negotiated Care Plan) and approved treatment plan.

Implement the ADL’s as directed by NCP for each resident, such as bathing, medication assistance and/or delegation, dressing, meal assistance and feeding, taking vital signs, turn and reposition in bed, assist with transfers, lifting and mobility, examining clients for bruises, injuries and/or wounds.

Perform normal housekeeping chores in the home (i.e. laundry, cooking, cleaning, daily wash tiled/linoleum floors and vacuum all carpeted areas, etc.) as directed by the Resident Manager/Provider. Be safe at all time in all your activities. Unsafe activities are cause for investigations and disciplinary actions.

Supervise residents to ensure adequate safety, comfort and health care and well being during your shift.

Perform Post Mortem Care when residents reach end of life.

Maintain resident confidentially and privacy.

Maintain appropriate staff/resident relationships.

Acknowledge the needs, wants, and rights of residents with dignity and respect.

Arrange transport for residents to and from AFH activities, as directed by the manager.

Accompany residents on outings, appointments and activities as directed

Complete all forms, reports, and data sheets as required by the Resident manager.

Safeguard all AFH and resident property.

Be aware and abide by all AFH policies, procedures, and practices.

Report promptly instances of injuries, behaviors, abuse, and safety hazards, etc. to the Resident Manager/Provider.

Maintain an ongoing inventory of resident's personal property.

Keep all hazardous materials, medications, etc. in the designated locked location. Keep locked cabinets locked.

Perform other responsibilities as defined by the Resident Manager.

Report to work on time.

Work independently and be available to support the AFH any shift and weekends as scheduled.

While on duty staff and/or management is not allowed to partake of or have any illegal drugs or alcohol, nor must they be on the premises while under the influence of illegal drugs or alcohol.

Drug free work environment and pass a drug test

Staff must be awake and alert while on duty for all shifts.

Prepare meals according to the menu and clean up kitchen after each meal afterward.
