Growing together.Role Description:Responsible for ensuring the continuity of care in the outpatient setting utilizing the appropriate resources and patient quality outcomes. Facilitates continuum of patients' care, with a command-center “virtual hospital” hybrid team, utilizing advanced nursing knowledge, experience, and skills.If you are located in Washington state, you’ll enjoy the flexibility to work remotely as you take on some tough challenges.Primary Responsibilities:

Provide nursing care to patients remotely and leverage clinical expertise through video conferencing, phone calls and other forms of telehealth technology

Navigate patients and families across the continuum of hospital at home care, monitor patient health status and progress, identifying and addressing any concerns promptly

Provide protocol-driven assessment and interventions as appropriate in the virtual setting

Facilitate admissions to hospital at home care, identify and escalate potential health concerns to appropriate healthcare/brick and mortar Providers promptly

Collaborate with in-home clinicians, such as Advanced Practice Providers, Registered Nurses, home health aides, physical therapists, and external vendors, to coordinate care activities

Job Scope and Guidelines:

Provide patient education and support on various health conditions and self-management strategies

Document patient-care activities in the patient’s electronic medical record

Problem solves issues independently as well as work with teams collaboratively in situations that require assessment, decision-making within the framework of established protocols, excellent listening and communication skills, knowledge of computers, critical thinking skills and the nursing process

Perform assessments and interventions within defined scope of practice as a registered nurse, develop and implement individualized care plans based on patient needs and goals

Collaborate with and escalate concerns to licensed healthcare providers as needed to ensure patient safety and optimal outcomes

Participate in team meetings and training sessions to stay informed about best practices and new technologies.

Graduation from an accredited school of nursing, preferred Bachelor of Science degree in Nursing, Undergraduate (Associate) degree, or equivalent experience

Current, unrestricted Washington State nursing license (RN) or Multistate license

3+ years of experience working inpatient within the last five years

Proficiency with: MS Office suite (Word, Excel, PowerPoint, Teams), video conferencing platforms (Zoom), EMR systems, and other virtual healthcare tools is crucial

Inpatient Acute Care, Medicine with IV, Home Based Care, and/or ER experience

Preferred Qualifications:

Specialty fields experience such as Medical/Surgical, Telemetry, Critical Care, oncology, or geriatrics
