Outreach FCS Liaison Coordinator

The Outreach FCS (Foundational Community Supports) Liaison will oversee the implementation, maintenance, and growth of FCS billing and Care Coordination in the Outreach Department. The coordinator will work closely with Outreach Case Managers and Specialists to provide guidance on integrating FCS billing and Care Coordination into the department’s day-to-day operations. The coordinator serves as the primary contact between Outreach and Care Coordination teams, acting in the role of a liaison. This position will also provide comprehensive care coordination at a reduced caseload. This will be a full time position at 37.5 hours per week.

JOB DUTIES and RESPONSIBILITIES INCLUDE:

1. Coordinate with FCS Coordinator (and other departments as needed) to provide up to date training on FCS/Care Coordination for Outreach Case Manager(s), Specialist(s), volunteers, and interns. Provide group and 1-on-1 trainings to outreach team as needed.

2. Co-develop, maintain, and update as needed, procedures and guidelines surrounding FCS/Care Coordination for Outreach team with program manager and relevant agency partners.

3. Serve as primary contact for FCS/Care Coordination in Outreach department and for staff in other departments.

4. Supervise, train, and support outreach specialist(s), volunteers, and interns in collaboration with the program manager.

5. Oversee and coordinate lobby hours.

6. Coordinate client care across departments, as needed.

7. Review EmpowOR logs for the department and check fidelity to FCS requirements.

8. Data collection, analysis, and reporting of FCS activity for Outreach program.

9. Maintain surveillance of department fidelity to requirements.

10. Regularly participate in inter-department meetings and agency committees related to FCS/Care Coordination to ensure Outreach is up to date in relevant. Provide report outs at Outreach program meetings.

11. Coordinate care that is safe, timely, effective, efficient, equitable, and client-centered. Help clients achieve wellness and autonomy.

12. Accept case assignments in a timely manner, review case progress, and determine program exits.

13. Advocate and refer for services, including financial assistance, tax assistance, legal aid, housing, job placement, education, primary healthcare, mental health, substance treatment, and other client-identified needs.

14. Educate and inform applicants of program requirement and responsibilities.

15. Work collaboratively with a client to identify goals, steps to achieve goals and resources needed to achieve self-sufficiency.

16. Daily data entry into EmpowOr and HMIS relevant to their clients.

17. Meet and maintain pre-established caseload and billable units.

18. Attends Community Action staff meetings and other meetings and trainings as required.

QUALIFICATIONS INCLUDE:

Education & Experience

High school diploma or equivalent required. Degree in Human Services or related field desired.

2 years related professional experience or a related peer certification. Lived experience with demonstrated success in self-advocacy may substitute.

Lived experience with homelessness and factors associated with housing instability preferred.

Supervisory experience preferred.

Or a combination of education and experience providing the knowledge, skills, and abilities to successfully perform the work

License(s) & Certification(s)

Must have valid driver license and auto liability insurance if personal vehicle is used for work-related travel.

Certified Peer Counselor Certificate preferred.

Training necessary (within 1 year of hire): Trauma-informed care, motivational interviewing, Blood borne Pathogens, First Aid/CPR, Safety, Mandated Reporting, Confidentiality, HIPAA, Crisis Intervention, and De-Escalation.

Skills and Abilities

Spanish/English bilingual helpful, but not required.

Demonstrated ability to lead and support others.

Solid observation, analytical and intuitive abilities.

Ability to set boundaries, resolve conflict, problem solve, and de-escalate issues.

Ability to work independently, as well as, part of a team.

Excellent organization and time management skills.

Ability to maintain confidentiality.

Ability to pass and maintain a background check.

Proficient in Microsoft Office Suite applications, including Excel, Word, PowerPoint, and Outlook programs.

Must have strong and effective communication skills (oral and written).

Working knowledge of standard office procedures and technologies (phone, computer, printer, photocopier, scanner, fax machine) is needed.

Demonstrated ability to work harmoniously with people from varied cultural, socioeconomic, educational and experiential backgrounds.

WORKING CONDITIONS / PHYSICAL REQUIREMENTS: Work may be performed in an office environment or outside street environment. May be required to sit, stand, or walk for extended periods of time. Sufficient mobility is required for the use of office equipment and driving. Hearing and communication ability must be sufficient to perform essential job functions. May be frequently exposed to situational, environmental or health hazards, such as mental health issues, drug abuse/paraphernalia, domestic violence, blood, fecal matter, parasites, and communicable disease. Travel within and outside Agency’s service area will be required for outreach, meetings, training and other job-related activities.

COMPENSATION AND BENEFITS:

Starting wage between $23.41 - $25.85 per hour (DOE)
