Rehab unit coordinator (RN)

Description:

As a rehab unit coordinator at CHCC, you will lead nursing staff, ensure assignments are balanced, coordinate with the interdisciplinary staff and help deliver patient-centered care in a professional, well-respected skilled nursing center.

Job functions:

24-hour responsibility for assigned care unit.

Ensures daily observation and assessment of each resident; directs and participates in resident care.

Ensures all aspects of the care plan are properly implemented.

Chart progress notes, including alerts, Medicare, doctor visits/new orders, care plan changes, etc.

Actively supports nursing staff as needed.

Assures resident safety.

Maintains daily communications with the director of nursing.

Leads the unit staff by ensuring proper orientation and training, addresses performance concerns.

Completes the unit licensed staff annual performance evaluations and coordinates nursing assistant reviews with licensed staff.

Serves as a role model and provides leadership in problem-solving among unit staff. Creates and promotes an atmosphere of respect, personal integrity and teamwork.

Ensures facility policies and procedures are followed and updated as appropriate.

Coordinates and ensures completion of resident restorative program for assigned unit.

Attends meetings with the director of nursing services; performs special projects as assigned by the director of nursing.

Qualifications:

Washington state RN license in good standing is required.

Two or more years of experience working as a licensed nurse in a long-term care setting.

Experience in IV therapy, starting peripheral IV lines, administering IV medication and performing blood draws for lab work.

Understanding of the MDS process and experience with Medicare.

Skills:

Knowledgeable and supportive of CHCC mission and philosophy.

Knowledge of state and federal regulations pertaining to long-term care.

Strong leadership, supervisory and teaching abilities preferred.

Exhibit strong assessment skills.

Competent in the nursing skills and procedures required on the nursing unit.

Excellent communication skills (verbal and written) and ability to work with diverse populations.

Works alongside employees to promote a team effort and maintain the highest quality resident service.

Work schedule:

Typically Monday through Friday, 8 a.m. to 5 p.m. On-call rotation is also required, as needed, to provide 24-hour coverage.
