POSITION SUMMARY

Development and implementation of individual program plan in accordance with applicable state and federal regulations. Coordinate all habilitation direct care services as defined by State and Federal regulations provided to residents within the facility.

QUALIFICATIONS

The QIDP is a healthcare professional who possesses excellent written and oral communication skills, excellent organization and problem solving skills. Should demonstrate an excellent ability to manage effectively, foster positive relationships, basic computer skills which are very essential to this position. Must possess a valid driver’s license.

EDUCATION

The QIDP must have a Bachelor’s degree in health or related field, with a minimum of 2 years of experience working with people with developmental or psychiatric disabilities one of which is in a supervisory capacity.

RESPONSIBILITIES

1. Ensure that the facility is in compliance with all active treatment regulations in providing habilitation services.

2. Coordinate efforts of the Interdisciplinary Team and program staff to ensure that each individual is receiving optimum services, which meet his/her individual needs.

3. Develop, implement, review, and document the residents' individual program plans.

4. Assist Administrator in coordinating all referrals, admissions, transfers, and dismissals of individuals from the facility.

5. Implement and maintain the facility's resident file system.

6. Serve as a member of the Admissions Team and provide necessary assessments and documentation.

7. Conduct staff training.

8. Maintain each individual’s finance and fund accounts, including balancing checkbooks.

9. Serve as the liaison with families/guardian, day training programs and the community.

10. Plan daily individual programs and activity schedules.

11. Participate/chair required committee meetings.

12. Perform other duties as requested by the Supervisor

13. Be able to cover shifts with the persons' in the case where there is no staff or the staff is absent.

JOB CONDITIONS

Minimal lifting of office records and printouts. Primarily, a desk job, which involves sitting, standing, stooping and walking. Excessive telephone communication and computer use. Traveling by car or airplane to local, out-of-town or state seminars, conferences or meetings. Must be able to read 12 point or larger type. Must be able to hear and speak in a manner understood by most people must be able to communicate both verbally and in writing.
