Follow an established plan-of-care implemented by the RN Case Manager. Assesses symptoms for immediate nursing interventions according to standing Hospice and/or M.D. orders, admissions, process referrals and evaluations of potential patients. Pronounces death according to regulatory guidelines. . Responsible for ensuring that care provided to patients is of excellent quality, meets home care rules and regulations, and follows physician’s orders. All employees, if applicable, are expected to adhere to the BSHR Automobile Safety Program per policy.On-Call availability will be Friday at 12PM until Monday 8AM .Essential Functions:Participates in on-call duties as defined by the on-call policy.Provides professional nursing care by utilizing all elements of nursing process.Makes home visits to assess and evaluate patient status.Makes necessary revisions as patient status and needs changeTeaches the patient and family/caregiver self-care techniques as appropriate.Assists with providing end-of-life care comfort measures as identified in the patient plan of treatment.Works in concert with the hospice interdisciplinary group members.Job Requirements:

Graduate of an accredited school of nursing. BSN Degree preferred.

Current licensure to the practice as a Registered Nurse in the Commonwealth of Virginia.

Current CPR certification

Valid Virginia Driver License and an acceptable driving record.

At least one year of nursing experience preferably in hospice, home health, med-surge or ICU
