LPN - Case Manager

Licensed Practical Nurse (LPN) - Case ManagerDays (Monday-Friday, 8a-4:30p, occasional weekends, no call)Position Summary:· Consults with nursing staff and multidisciplinary team regularly to evaluate patient's status and appropriateness of medical care, including admission, length of stay, transfer and discharge.· Monitors patient and family satisfaction. Responds to questions and complaints from patients, family members, and payors regarding care.· Participates in discharge planning including coordinating patient transfers to other facilities and coordinating community resources. Provides discharge education and resource referrals to patients.· Performs chart review to identify actual or potential issues with service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement.· Reviews patient records and completes case management review process within the prescribed time frame outlined in policies and procedures.· Monitors patients’ coverage for continued hospital stay for appropriateness within the prescribed time frame outlined in policies and procedures.· Determines appropriateness of admission or continued stay as indicated by established criteria and communicates with physicians to assure guidelines are met.· Identifies high-risk/long-stay patients and communicates with physicians/patients/families to develop and implementplan for discharge.· Develops discharge plan within one working day of admission and starts initiation process with input from patient and/or family.· Utilizes community resources/financial assistance/home health agencies, durable medical equipment supplies, long term/short term, and residential care to impact discharge process.· Identifies areas of documentation improvement needed by physicians and provides appropriate physician education.· When appropriate and assigned by the Director, gathers data for CORE measure initiatives, infection control, Joint Commission guidelines, Medicare guidelines and comprehensive chart reviews.· Initiates and participates in interdisciplinary communication for patient’s case management/discharge needs.· Initiates communication with attending physician and implements peer-to-peer consultations to prevent denials of hospitalizations.· Maintains current knowledge of Medicare, Medicaid, and other healthcare insurance regulations and complies with these standards related to readmission reviews, required peer reviews, etc.· Initiates appeal for denials by compiling data to provide appropriate information for review.Who We Are:People are our passion and purpose.Come work where you are appreciated for who you are not just what you can do.
