Initial patient and family assessment to determine home care needs

Comprehensive physical assessment and medical history review

Ongoing re-evaluation of patient's nursing needs

Development and revision of care plans as per patient's health status

Nursing diagnosis based on health assessment data

Goal setting using nursing diagnosis for therapeutic, preventive, and rehabilitative actions

Inclusion of patient and family in treatment planning and goal setting

Administration of physician-prescribed medications and treatments

Patient education on healthcare management according to the care plan

Discharge planning identification as needed

Adherence to home health agency policies and protocols

Participation in case conferences, team meetings, and educational in-services

Compliance with professional standards, federal, state, and local regulations

Completion of required training and competency programs
