POSITION: Case Manager DIVISION: Community Developmental ServicesCommunity Developmental Services is a Division of Washington County Mental Health Services. We provide comprehensive services to children and adults with intellectual / developmental disabilities, autism, and/or other pervasive developmental disorders.SUMMARY OF POSITION:Provide Service Planning & Coordination to individuals with intellectual disabilities so they may gain access to services and supports in order to achieve successful outcomes. Examples of service provision includes, but is not limited to, developing, and implementing a person-centered plan, coordinating medical and clinical services, establishing and maintaining a case record, coordinating staffing supports for community and employment services, overseeing housing/home supports, and coordinating crisis services as needed.You might be the right person for the Case Manager job if:

You’re a great communicator and collaborator – You find it rewarding to engage in a collaborative team process that allows you to use continual and active listening skills for the purpose of identifying the strengths of the individual as well as their current and future goals.

You are self-motivated & are a problem solver – You take initiative with tasks and can prioritize the needs of your caseload. There will be times, when you will be juggling competing priorities and will need to exercise good judgement and organizational skills. There is always supervisory guidance & support available when needed!

You are intrigued by the concept of empowerment and self-determination – You like educating and empowering individuals to play a role in their own life’s decisions and to be a part of community inclusion.

You find satisfaction in meeting deadlines – You like word processing and enjoy entering documentation into an electronic client health record in a timely way. This will be in accordance with agency policy and guidelines set forth by the Center for Medicaid and Home & Community Based Waiver Service guidelines.

You want to make a difference in a person’s life – you find reward in helping others to identify their strengths, needs, and unmet needs as they relate to Vermont’s Developmental System of Care Plan. A large part of this role is to assist individuals in the development of goals and strategies to increase skills, social connections, maintain career goals and overall health and wellbeing.

You enjoy supervising & coaching others – Providing supervision to direct care staff to effectively support the individuals we serve. This includes providing staff with tools they need to perform their jobs, providing them with training and helping them become a resource. If you don’t have experience in this area – that’s okay, we can provide comprehensive training including supervisory skills training!

QUALIFICATIONS:Bachelor’s degree in related field. QDDP & Supervisory status preferred with a minimum of 3 years’ experience working with intellectual disabilities.
