Position Summary:The Director of Pharmacy provides administrative and managerial leadership for the Pharmacy Department; supervises the pharmacy technicians in unit; does distribution system for specific hospital units and medication unit inspections; participates in multi-disciplinary patient care plans; assists in the control, ordering and wastage of controlled substances; assists in the day-to-day operations involved with the Omnicell system, maintains an optimal level of pharmaceutical inventory through application of effective purchasing procedures with the appropriate contract vendorsReports to: Chief Executive OfficerFLSA: ExemptOther Duties:

Compounds and dispenses prescribed medications and other pharmaceuticals for patient care by performing the related duties

The Pharmacist maintains established procedures concerning quality control, security of controlled substances and disposal of hazardous waste.

Enters data into the computer system such as patient, prescribed medications and cost and maintains patient profiles and charge system.

The Pharmacist will instruct student and other medical personnel on matters pertaining to the Pharmaceutical Services Department. Other Duties as assigned.

Reviews prescriptions issued by physicians or other authorized prescribers to assure accuracy and determines formulas and ingredients needed.

Monitors all posting onto the patient profiles in order that accuracy, drug allergies, drug interactions, duplicate therapy, rational drug therapy and incompatibilities may be realized and better patient care received.

Checks patient medication cassettes prior to delivery to patient care areas to ascertain that the correct drug, dosage form, schedule and strength is being dispensed.

Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.

Prepares IV admixtures and extemporaneous parenteral products under aseptic conditions.

Directs Pharmaceutical Services personnel engaged in admixing, packaging and labeling pharmaceuticals.

Answers questions and provides information to physicians, nurses, patients and other healthcare professionals on the appropriate use of medications, drug interactions, side effects, dosage, storage, etc.

Enters data into the computer system (Pharmacy program) such as patient, prescribed medications and cost and maintains patient profiles and charge system.

Instructs students and other medical personnel on matters pertaining to the Pharmaceutical Services Department.

Ensures the maintenance and proper operation of equipment and supplies in the work areas.

Director Role Responsibilities:

Provide administrative and managerial leadership for the pharmacy department.

Maintains an optimal level of pharmaceutical inventory through application of effective purchasing.

Maintains a formulary which promotes patient safety, improves financial performance and reduces waste.

Maintains pharmacy practice standards which meet state and federal law

Compounds medications using standard formulas and processes such as weighing, measuring and mixing ingredients

Reviews prescriptions issued by physicians or other authorized prescribers to assure accuracy and determines formulas and ingredients needed.

Maintain an automated dispensing system to prevent errors, capture 100% of billing, and provide accurate real time billing.

Maintains omnicell system

Leads drug diversion program and training for new hire orientation

System Wide Responsibilities:Maintains compliance with regulatory requirements, including all state, TJC and Federal regulations in their area of practice.

Attends and participates in department staff meetings; reads and signs all meeting minutes as acknowledgement of information received; demonstrates compliance and knowledge of new changes within department and hospital.

Consistently supports the mission, vision and goals of the organization. Acts in a positive, proactive manner to support department and hospital efforts in the community and with co-workers.

Complies with annual TB and Fit Testing requirements as it related to the job.

Has demonstrated an awareness and implementation of National Patient Safety Goals as applicable in their area of practice.

Knows, understands, and adheres to system/hospital/department policies and procedures.

Demonstrates and takes responsibility for workplace safety. Exhibits knowledge regarding the hazard and injury reporting procedures.

Complies with Confidentiality/ HIPAA requirements and observes patient privacy needs.

Understands Cultural/spiritual diversity issues as they relate to the job; is cognizant of patient's cultural differences and includes spiritual health as a component of patient care.

Practices proper infection control techniques and understands importance of following IC requirements.

Attends mandatory in-services, staff meetings and pertinent seminars.

Completes mandatory annual education requirements.

Maintains all licenses and certification in good standing and active.Position Requirements:

Doctorate of Pharmacy degree or Registered Pharmacist (RPh) or equivalent. Graduated from an accredited School of Pharmacy.

Must have graduated from an accredited Pharmacist curriculum

Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.

Basic Life Support (BLS) within 60 days

Utah State Pharmacist license
