creating a partnership relationship with providers and other initiatives and
implementing the concept of a Medical Home for our members to achieve this
fully integrated and accountable system of care.Primary duties mayinclude, but are not limited to:
Creates partnerships with Provider groups toeducate on HEDIS measures and best practices for gap closure
Conducts HEDIS medical chartreviews.
Review and perform qualityaudits of supplemental data and Clinical Practice Guidelines
Providesongoing practice support and sustains the partnership.
Establishesregular meetings to track progress.
Improvesthe understanding and sensitivity to the unique needs of this Medicarepopulation.
Supportsthe annual HEDIS Medical Record Review (MRR)
Professional audience presentation experience
Position Location –Texas onsite
Requiresan AS/BS in nursing; 5 years of managed care experience; 2 years of clinicalexperience; 2 years of professional presentations to small and large audiences;
3years of HEDIS experience- required
Current,unrestricted RN license
NCQAStandards, HEDIS audits, site reviews and medical records reviews.
Bilingual-Spanish, Chinese, Vietnamese or Korean- highlypreferred
Experiencesupporting Diverse population
Must be able to travel
