Texas Health Resources seeks to hire a Community Health Worker PRN to work “As Needed” in their Community Health and Benefit Administration Department.Position will primarily work in the Denton AreaSalary range is Minimum $17.61/hr. to Maximum $28.08/hr. – based on relevant experienceWork Schedule:

PRN “As Needed”

Primary hours will be from 8:00 AM – 5:00 PM either working five, 6.5 hour days or four, 8 hour days.

Job Description:The Community Health Worker (CHW) will serve as educator/coaches for lifestyle changes and disease management. To effectively meet the clients where they are, the CHW will conduct home visits to assess changes that need be made and social determinants of health barriers that may need to be overcome. They will also provide education and encouragement through home visits thus alleviating the transportation barriers. Navigation will be provided for both healthcare and individual needs. The CHW will assist clients in completing applications for services addressing the basic needs of food, clothing and shelter in addition to Medicaid and other Market Place insurance coverage as appropriate. Health literacy will be improved through both one-on-one and group education on risk factors for diabetes, lifestyle changes, self-management techniques and other applicable topics. Through their work, the CHW will break down the social determinants of health barriers to improve the lives and health of the people in the community they serve.Essential Functions:Coordinate Community Health Improvement Activities

Participate in program development and planning

Develop and effectively manage program timelines to ensure timely completion of program deliverables

Monitor program activities and reporting to ensure accuracy in outcome measures

Facilitate small- and large-group education

Establish and manage relationships with community members, organizations and coalitions

Identify strategic partnership opportunities and promote relationship-building

Disseminate programmatic and marketing materials

Develop presentations and other materials for use during program-related events

Serve as programmatic liaison to internal and external stakeholders

Patient Support

Responsible for establishing trusting relationships with patients and families

Provides ongoing follow up via phone calls or home visits as appropriate

Navigates patients and families through medical and non-medical support services

Conducts eligibility, enrollment and follow up for referred medical or non-medical support

Helps patients set personal care plans and attend appointments

Works closely with medical care team to ensure coordinated care

Serve as liaison to internal and external resources through TH network

Report activites through CBISAQualificationsThe ideal candidate will possess the following qualifications:

High School Diploma required

Bachelor's Degree preferred

Successful completion of a Community Health Worker formal training program such as from a community college or other education institution preferred

1 year experience coordinating volunteer services/programs, public speaking, completing activity reports, recruiting volunteers, and partnering with external organizations required.

Skills :

Strong verbal and written communication skills

Ability to maintain composure in stressful situations, ability to problem solve, time management skills, detail-oriented

Ability to work independently as well as with others

Strong organizational & prioritization skills

Ability to motivate and inspire others

Ability to complete required reports
