Work location: Primarily in the Denton area

Work hours: Primarily Monday - Friday with flexibility on schedule (7:30am-4:00pm, 8:00am-4:30pm, or 8:30am-5:00pm)

This is a grant funded position and subject to the award and continuation of the contract

Community Health Department Highlights· Facilitate access to health services for underserved community members· Role is connected to system changes to address social determinants of health· Empowered to be a servant leader in Community HealthHere’s What You NeedEducationH.S. Diploma or Equivalent Required andOther Successful completion of a Community Health Worker formal training program such as from a community college or other education institution PreferredExperience6 Months Customer service experience either face-to-face or by phone RequiredLicensures and CertificationsBCLS – Basic Cardiac Life Support within 60 days RequiredSkillsStrong verbal and written communication skills, ability to maintain composure in stressful situations, ability to problem solve, time management skills, detail-oriented, ability to work independently as well as with others, strong organizational & prioritization skills, and ability to motivate and inspire others, ability to complete required reports.What You Will DoProvide Client Support:· Responsible for establishing trusting relationships with patients and families.· Assists patients with personal goal setting and care plans.· Supports clients with meeting personal goals including but not limited to attending appointments, providing support for scheduling meetings/appointments with agencies and resource providers.
