We are a home health agency based in Garland, Tx that provides home health care to patients throughout the DFW area. We strive to provide the highest quality and comprehensive health care to our patients and families.
JOB DESCRIPTION
LVN-home health care may include some or all of the following tasks:
Perform wound care according to treatment plan, assess and monitor wounds, clean and bandage wounds
Manage medications, give injections, pre-fill pill box per medication profile and instruct patient/caregiver on pill box management, administer prescribed medications as directed by physician and in accordance with nursing standards
Provide effective communication to patient/family, team members, physicians, and other health care professionals to ensure coordination of services
Monitor and assess patients at home, monitor vital signs and perform skilled tests, monitor patients at home skills such as: Taking medicines and activities of daily living
Perform related patient care functions such as taking temperature, pulse, respiration and blood pressure readings
Assist patients with wellness skills and the ability to function well at home
Use patient education, active listening, and reinforcement of good wellness skills
Provide health teaching and assist the clients and families in learning self-care techniques.
Communicate and document findings in a factual, concise and timely manner per agency policy.
Knowledge of trach care, peg care and vent care (preferred)
REQUIRED EDUCATION/EXPERIENCE:
Graduate of an accredited School of Vocational/Practical Nursing
Current State license as a Licensed Vocational/Practical Nurse
One year experience as a Licensed Vocational/Practical Nurse in a home care setting
Proof of current CPR
