Master's Degree in Social Work is required.

Three (3) years in hospital/medical social work is preferred.

One (1) Year discharge planning/care management is preferred.

Licenses and CertificationsLMSW - Licensed Master Social Worker is required upon hire orLCSW - Licensed Clinical Social Worker required upon hireCPR - Cardiopulmonary Resuscitation is required upon hireACM - Accredited Case Manager is preferredCCM - Certified Case Manager is preferredOther ANCC is preferredWhat You’ll DoYou’ll ensure patients are transitioned to appropriate levels of care and educate patients, caregivers, and the multidisciplinary team regarding available post-acute care services and needs. Execute and updates the discharge plan as needed.You’ll facilitate care conferences for complex transitions, placement and palliative care need and serve as a point of contact for all patient care stakeholders.You’ll participate in Joint Commission and other survey readiness activities and serves as a content expert on the following: compliance requirements for delivery of HINN, second IMM and MOON letters; Potential denials, avoidable days, and alternate level of care days; medical necessity, patient status and discharge criteria and clinical review staff requirements and communications.Do you still have questions or concerns?
