Pharmacy Technician Medication Reconciliation Job DescriptionWe are seeking a CPhT to assess medication history for patients in the ERResponsibilities

Conduct medication history assessments for new admissions, detailing each medication (drug, dose, route, frequency, PRN indication, last dose).

Document allergy history comprehensively (substance, reaction).

Verify medication compliance prior to admission.

Prompt patients for additional medications, including OTC products (herbals, vitamins, pain relievers, etc.).

Enter preferred outpatient pharmacies into the electronic health record.

Update medication histories in the electronic health record promptly.

Ensure accuracy and follow-up on incomplete information.

Maintain knowledge of brand/generic medication names, dosages, forms, and frequencies.

Communicate effectively with patients and healthcare professionals.

Collaborate with nurses, pharmacists, and prescribers for accurate assessments.

Use clinical judgment to verify medication history accuracy.

Navigate hospital tracking boards for patient selection.

Prioritize tasks in a high-volume environment.

Work independently and assist coworkers as needed.

Report medication history errors to supervising pharmacists.

Complete applicable reports and participate in performance improvement activities.

Precept pharmacy staff and students on medication interviews and documentation.

Maintain continuing education for certification and licensure.

Contribute to the safe and efficient operation of the Pharmacy Department.

Adhere to the Covenant Pledge of Excellence, policies, procedures, and safety standards.

Complete required education assignments and participate in quality improvement initiatives.

Perform other duties as assigned.

Additional Skills & Qualifications

Ideally 3-5 years of pharmacy technician experience, but flexible!

Proficiency in using computer systems for data retrieval and data entry.

Pay and BenefitsThe pay range for this position is $19.00 - $20.00/hr.Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
