Job Title: Pharmacy TechJob DescriptionWe are seeking a CPhT to assess medication history for patients in the ER. The Pharmacy Technician will perform medication history assessments for new hospital admissions, ensuring accurate documentation and verification of all medications. The role involves working closely with healthcare professionals to provide comprehensive and precise medication information.Responsibilities
Perform medication history assessments for new hospital admissions, providing complete information regarding each medication identified.
Obtain allergy history and ensure complete documentation.
Determine compliance with medication consumption prior to admission.
Prompt patients for additional medications, including over-the-counter products.
Enter patients' preferred outpatient pharmacies into the electronic health record.
Navigate the electronic health record database to enter or update medication histories.
Ensure accurate work with careful attention to detail and persistence in follow-up.
Communicate effectively with patients of all education levels and health professionals.
Work in consultation with nurses, pharmacists, and prescribers to complete medication history assessments.
Use clinical judgment to determine the accuracy of medication history lists.
Navigate hospital tracking boards to determine patient selection.
Prioritize work in a high-volume environment.
Work independently and as a team player to assist coworkers.
Take initiative in performing miscellaneous functions within the department.
Report errors or concerns about medication history to a supervising pharmacist.
Review and complete applicable reports.
Participate in performance improvement activities and volunteer for projects.
Precept pharmacy technicians, pharmacy students, pharmacists, pharmacist residents, and nurses.
Maintain continuing education for certification and licensure.
Contribute to the safe and efficient operation of the Pharmacy Department.
Act in a manner consistent with the Covenant Pledge of Excellence.
Follow policies, procedures, and safety standards. Complete required education assignments annually.
Work towards achieving goals and objectives and participate in quality improvement initiatives.
Essential Skills
CPhT certification
Customer service skills
Experience in retail pharmacy
PTCB certification
Excellent knowledge of brand and generic medication names
Proficiency in using computer systems for data retrieval and data entry

Additional Skills & Qualifications
Ideally 5+ years of pharmacy technician experience or 2+ years of pharmacy technician experience and a Pharmacy Technician Certification Board Medication History Certificate
Previous outpatient pharmacy experience, including prescription transcription duties
