Summary Supervises and coordinates the activities of personnel in Diabetes Education Center to maintain continuity of care in conjunction with Baptist mission. Works to ensure standardization of systems, identification of efficiencies, integration of performance improvement strategies that support facility goal attainment while fostering patient, physician and employee satisfaction. Works collaboratively with other departments to operationalize service line development. Incumbent is accountable for departmental management. This position is under the general direction of the Clinical Director.Responsibilities
Assists with the development of operational plans by assessing and evaluating department's service to meet the needs of the patients and increasing the department's contribution to the overall organizational outcomes.
Supports and defines policies that promote patient's fundamental right to considerate care that safeguards their personal dignity and respects their cultural, psychosocial and spiritual values in an effort to meet patient's care needs and preferences, adhering to regulatory agency guidelines.
Assures competency of staff to assess the physical, psychosocial, spiritual, educational and environmental status of the child, adolescent, adult and older adult patient in a systematic and continuous manner.
Organizes, directs, and provides patient care, coordinating services to provide for quality patient outcomes.
Promotes ongoing assessment of patient/family's response to treatment evaluating the effectiveness of the patient's care plan.
Develops, provides, and coordinates education to patient/families and staff to enhance safety, continuity of care and knowledge of health.
Participates in performance improvement activities by identifying areas of opportunity to ensure quality is consistent with established guidelines.
Maintains a safe, functional and effective environment for patients and staff.
Practice reflects knowledge base of current medical/clinical literature, resources and standards of practice.
Actively participates in promotion of Service First activities and goal achievement.
Works with internal departments and external referral sources to promote service line.
Performs other accountabilities/responsibilities as directed or assigned.
Requirements, Preferences and Experience EducationPreferred: Bachelor degree preferredMinimum: Graduate of approved School of Nursing or DieteticsExperienceMinimum: Two years' experience in clinical setting with experience in the specialty area of diabetes. Demonstrated leadership skills.Licensure, Registration, CertificationMinimum: RN or RD in the state of TN. Certified Diabetic Educator or in the process of obtaining CDE- meet the requirements to test for certification.
