We want your knowledge and expertise!Job Summary and QualificationsThe Quality Improvement (QI) Coordinator is responsible for the coordination, implementation, improvement, and successful achievement/compliance of core measure indicators. Coordinator is involved in patient education, physician and clinician collaboration regarding treatment plan, development and implementation of disease-specific tools for core measure diagnoses, clinical documentation, data abstraction and monitoring of core measure data. Liaison between physicians, nurses, administration, patients, and external bodies for core measure compliance. Responsible for quality improvement aspects such as data management, analysis, improvement strategies, and presentation to applicable departments, committees, or groups. Thorough understanding of the core measure intent and quality improvement fundamentals essential. Ability to establish good working relationships with physicians and nurses. Ability to establish rapport with patients to educate them about disease process, clinical care, discharge instructions, and lifestyle changes necessary to improve their health status. The QI Coordinator is also responsible for the coordination of monitoring and reporting of medical staff quality improvement for re-credentialing, data abstraction, submission, and analysis of core measures. Assists with the coordination of the QI program throughout the hospital via interdisciplinary measurement, analysis, and actions to improve services. Maintains systematic data collection and flow of information between departments and into applicable committees.Participates in Performance Improvement activities to assure continuous Quality Improvement as appropriate. Maintains cultural and diversity awareness when dealing with patients, visitors, and other customers. Promotes a supportive, team-work oriented environment. Fulfills departmental responsibilities with the long-range plan, mission, vision, and values at Horizon.· Current TN RN license required.· Certified Professional in Healthcare Quality (CPHQ) preferred.· 1-2 years of experience in a management position is preferred· Excellent interpersonal skills to successfully interact with hospital personnel and physiciansTriStar Horizon Medical Center is an accredited, 150+ bed regional medical center.
