As aPopulation Health Advisor, you will lead and support the growth of ACO/CIN and population health initiatives. Your responsibilities include tracking and analyzing clinical quality measures and government requirements (e.g., MIPS, MSSP, ACO Promoting Interoperability), educating ACO/CIN providers on proper documentation and coding for accurate patient risk profiles (HCCs or other), and overseeing strategic initiatives like primary care workflow improvements, post-acute care, and palliative care, with a focus on care delivery, customer service, and financial sustainability.

Responsibilities:

Deliver comprehensive MIPS, ACO/MSSP, and HEDIS quality reporting for ACO/CIN contracts.

Develop and track performance improvement initiatives for clinical quality measures and primary care workflow optimization.

Supervise the improvement of ACO/CIN provider documentation and coding accuracy for risk adjustment.

Serve as a liaison between ACO/CIN leadership and practices, communicating strategic priorities effectively.

Manage and educate on ACO/CIN payer-specific cost, quality, and utilization reports.

Train ACO/CIN providers and staff on incorporating data and technology into daily workflows.

Oversee strategic ACO/CIN projects related to quality improvement, risk adjustment, network development, and post-acute care optimization.

Facilitate technology projects within the ACO/CIN, such as EMR integration for quality reporting.

Collaborate with care management or care navigators across all lines of business.What you’ll need:

Education:Associates degree in Business, Analytics, Nursing or related field.

Experience: Two years of experience in the ambulatory healthcare setting including participation within quality improvement and/or population health.
