Local Non-Profit In- Home Care Provider is seeking LPN's and RN's
We are a non-profit organization that is on a mission to build a strong team of nurses to help assist us with providing nursing care to the elderly, sick and disabled children within the community. Our goal is to help maintain independence and comfort of the client's home. We are looking for kind -hearted nurses that has the spark and instinct to care for those in need and to leave a positive effect on people's lives.
LPN/RN: The nurse will be responsible for traveling to clients that are authorized through the CLTC Medicaid Program to receive nursing care in their home. Type of nursing care will include, but not limited to administering at home IV’, skin/decubitus Care, tube feeding, catheter care, tracheostomy care , wound care, ventilator care, to those with chronic illness to non- acute, long term medical conditions or those suffering from profound, permanent functional limitations due to stroke, injury, or congenital disorders. Key services include close medical and nursing supervision, medication and symptom management, mobility support, continence management, security and supervision, documentation, competency training evals, monitoring of PCA abilities to follow client service plan.
