They work closely with the Clinical Manager to oversee and coordinate the daily activities, ensure quality patient care, and promote the success of the program. The Nurse Navigator provides guidance, logistic support, and oversight to the interdisciplinary team, maintains compliance with regulatory standards, and contributes to the program’s continuous improvement initiatives. They function as a clinical resource for staff, assist with the clinical team’s assigned shifts, and intervene in situations requiring immediate action. Coordinates and facilitates patient appointments, including those made by internal RSFH services as well as contracted vendors to streamline plans of care, interrupt patterns of frequent acute hospital or emergency department use, and promote health status through patient self-management skills. The Nurse Navigator provides nursing care for patients following established standards and practices following the South Carolina Nurse Practice Act (RN) and following hospital standards and professional practice.Minimum Qualifications :Education: BSN from an accredited nursing school.Experience: At least three years of recent direct patient care experience as a clinician; Experience in home health preferred but not required.Licensure/Certification: Currently licensed as a Registered Nurse in the state of South Carolina or holds a current compact/multi-state license as a Registered Nurse in a recognized NCSBN Compact State and is not a permanent resident of SC. Skill in applying and modifying the principles, methods and techniques of professional nursing to provide on-going patient care. Skill in identifying problems and recommending solutions. Skill in preparing and maintaining records and writing reports. Skill in establishing and maintaining effective working relationships with patients, hospital, medical staff and the public. Ability to analyze situations and apply critical thinking. Ability to communicate clearly. Ability to react calmly and effectively in emergency situations. Must have a proven track record of excellent communication skills with patients, physicians, and other healthcare providers. Health plan and medical insurance knowledge. Fo r positions in the Stroke Center of Excellence : Thorough knowledge of adult neuroscience and stroke treatment, care and prevention. Thorough knowledge of current evidence-based practice and standards of care for neuroscience and stroke patient populations and maintenance of related educational and credentialing requirements.Contacts : Constant interaction with internal and external customers to include (but not limited to) patients, families, physicians, employees and visitors.Work Demands/Environment : Frequent standing, walking, bending, crouching, stooping. Frequent lifting/moving patients. Intermittent lifting, moving or carrying, pushing or pulling up to 50 lbs. Frequent use of finger/hand dexterity. Constant reaching with hands/arms. Frequent talking or hearing. Corrected hearing and vision to normal range. Exposure to blood, body fluids or tissue. Possible exposure to infectious materials, communicable diseases and/or other conditions common to a healthcare environment. Intermittent exposure to risk of exposure to blood-borne pathogens. Moderate noise level in work area. Requires work under stressful conditions, deadlines, or irregular hours.
