We are looking for a dedicated Discharge Planning Associate (Registered Nurse or Bachelor's in Social Work) to join our team at UPMC Williamsport and Muncy . This full-time position offers daylight hours from Monday through Friday, with occasional weekend or holiday coverage.Why You'll Love This Role:

Collaborative Team : Work alongside a dedicated Discharge Planning team to coordinate support services and resources, facilitating effective discharge plans that achieve optimal satisfaction, clinical, and financial outcomes.

Key Responsibilities:

Holistic Patient Care : Identify and address clinical, psychosocial, historical, financial, cultural, and spiritual needs to guide the planning process and achieve optimal outcomes.

Comprehensive Assessments : Conduct detailed assessments to understand medical and social factors, support systems, and potential barriers to discharge.

Efficient Discharges : Facilitate safe and efficient discharges by maintaining knowledge of area resources and ensuring appropriate post-hospital care arrangements.

Clear Communication : Engage in clear communication with patients, caregivers, and the interdisciplinary care team to develop and execute individualized discharge plans.

Liaison Role : Serve as a contact between hospitals, post-hospital care facilities, and physicians to ensure seamless transitions.

Shared Accountability : Demonstrate shared accountability in developing discharge plans with patients, caregivers, and team members.

Ethical Practice : Align your practice with the mission, vision, and values of UPMC, adhering to ethical standards and regulatory requirements.

Patient Advocacy : Advocate for patients' health, well-being, safety, and rights, ensuring access to necessary services.

Diploma or Associate's Degree in Nursing

OR

Bachelor's degree in social work or another health or human services field that promotes the physical, psychosocial, and/or vocational well-being of those being served (no license required).

Experience:

No experience in discharge planning/care coordination is required.

Clinical/patient-facing experience preferred.

Knowledge and Skills:

Excellent communication skills.

Proficient in planning/organization, follow-up/control, problem-solving, and self-development orientation.

Strong organizational behaviors/competencies.

Ability to apply principles of logic and critical thinking to a wide range of problems and deal with a variety of abstract and concrete variables.

Comfortable working both independently and as a team member.

Proficient computer skills.

Licensure, Certifications, and Clearances:Registered Nurses employed in this position are required to maintain active RN license.
