Full Time 40 HoursJob Description/Essential Functions:Horizon House is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues and substance abuse. With over 100 locations throughout Southeastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strengths and choices.

We are seeking a Mobile Psych Rehabilitation Specialist to provide skill/resource development, counseling and encouragement to clients as they move into vocational or community based activities of their choice; ensures that all clients receive the level of and/or have access to the services/supports they choose, and to monitor and track client progress of assigned caseload; participates in the development of individualized rehabilitation plans.This position receives general supervision from designated Manager and/or Mobile Psychiatric Rehabilitation Team Leader and some of their duties include:

Maintaining a close liaison with the local community and area service providers/agencies to facilitate program cooperation and provide for constructive input and/or alternative resources to current plans.

Maintaining and monitoring an assigned caseload of clients.

Participating in relevant training to enhance professional growth and development, with an emphasis on counseling, case-management and basic living skills.

Participating in the resolution of crisis situations; assist with identifying key individuals and resources that can provide direct support and/or immediate intervention during a crisis situation.

Job Qualifications:

B.A./B.S. in the Social Sciences, or a high school diploma and twelve credits in social sciences with two years related experience.

Very good communication skills.

Ability to work independently and as part of a team.

Detail oriented.

Scope:

Requires the ability to lift, carry, fold, kneel, stack, stoop and bend which may include direct intervention with clients.

Possess manual dexterity and fine motor skills.

Must be available for local travel and possess a valid driver's license with an acceptable record.

May require crisis intervention to emergency situations.

May be required to carry beeper during scheduled emergency on-call rotation.

May be required to report to work during emergencies including inclement weather.
