ID/DD CoordinatorApplyRefer a FriendBackJob DetailsRequisition #:195877Location:State College, PA 16801Category:Client ServicesSalary:$50,000 - $55,000 per yearPosition DetailsPosition OverviewThe ID/DD Coordinator is responsible and accountable for the coordination and administration of services for clients and their caregivers under the direct supervision of the Location or Executive Director.Essential Job Functions Provide initial evaluation to determine appropriateness and safety of client and environment. Follow policies and procedures and conforms to all applicable patient care standards, licensure regulations and state laws Provide a written summary for individual clients and services based on needs, goals, and expected outcomes in conjunction with the ISP to Supports Coordinators/Administrative Entities on a monthly basis Participate in the development of Individual Support Plan Maintain patient confidentiality Demonstrate an understanding of client's medical status and updates self on clients/family status as needed or requested Maintain effective communication between staff, supports coordinators, and family Review progress notes from paraprofessionals to determine appropriate service delivery Educate paraprofessionals on individual client ISP expectations Perform on site evaluation of employee performance at 90 days and annuallyRequirements Bachelor’s degree in Psychology or other social science or related field Current CPR certification Proof of eligibility to work in the United States Current completion of child abuse clearance and FingerprintingAdditional state specific requirements N/APreferences Minimum of one year of experience with ID/DD preferredOther Skills/Abilities Must be able to adhere to confidentiality standards and professional boundaries at all times Knowledge and understanding of compliance with adherence to regulations Ability to comfortably work with families with limited resources Quick-thinking and astute decision making skills Attention to detail Time Management Effective problem-solving and conflict resolution Ability to remain calm and professional in stressful situations Strong commitment to clinical excellence Excellent organization and communication skills Leadership skills Ability to train and supervise staffPhysical Requirements Must be able to speak, write, read and understand English Must be able to travel; company does not provide vehicles or transportation Frequent lifting, carrying, pushing and pulling of 25 pounds Occasional lifting, carrying, pushing and pulling of 50 pounds Prolonged walking, standing, bending, kneeling, reaching, twisting Must be able to sit and climb stairs Must have visual and hearing acuity Must have strong sense of smell and touchEnvironment Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions Possible exposure to blood, bodily fluids and infectious diseasesOther Duties Please note this job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employee for this job.
