UPMC Canterbury Place is hiring a Full-Time Social Worker to join our team!The hours will be Monday - Friday 9 AM - 5:30 PM.Apply Today to Learn More!Responsibilities:

Continual documentation in reference to discharge plan revisions, room moves, Psychiatric visits and any other pertinent resident/family issues, all to be documented within 48 hours of the event/phone conversation.

Initiate advance care planning with a residents/family members/significant others

Provide support to resident and/or families as needed to assure that psychosocial needs are identified and met.

Coordinate all Family Orientation meetings to occur the first Monday or Thursday after admission.

Actively participate in in-service and continuing education programs for personal benefit as well as to provide up to date educational programs for residents, families, staff, and the community concerning the aging process and care of the elderly. Facilitate in-services for staff according to the current mandatory schedule.

Perform all investigations deemed necessary by the DON and/or Personal Care Home Administrator.

Implement and document a plan of care to meet the psychosocial needs of residents in accordance with policies and procedures of Canterbury Place and standards established by regulatory agencies.

Complete Long Term Care Insurance forms and submit in a timely fashion to the Business Office.

Perform related social service responsibilities as requested by the Personal Care Home Administrator.

Consult Ethics Committee as needed

Complete all proper documentation for reported missing items and perform the follow-up investigations in a timely manner.

Coordinate psychiatric care of residents with attending physician and psychiatrist.

Coordinate all room changes as necessary for the health and well-being of the residents.

A Social History is to be completed on the designated form and placed on the medical record within 48 hours of admission.

Attend daily team meetings to enhance successful communications with the DON, Marketing Manager, and Personal Care Home Administrator to assist in maintaining full occupancy of the Personal Care and Harmony Hall.

Coordinate all Psychiatric commitments to inpatient facilities.

Establish and maintain an atmosphere of caring, concern, and support for residents, family, staff, and peers, demonstrating on a consistent basis the behaviors, which comprise The Basics of Service Excellence.

B.A. Degree in Social Work.

2 year's experience in social work in a health care setting is required.

Knowledge of available services and health care delivery systems for the elderly.

Excellent interpersonal relationship skills.

Excellent independent judgment in order to prioritize work demands.

Ability to work as a team with residents and/or their families, visitors, staff and peers is required.

Tact and diplomacy in dealing with staff, residents, and visitors are necessary at all times. Licensure, Certifications, and Clearances:
