Deliver/Observe and educate person served on medication(s)/side effects and use
Provides outreach, case management, counseling, advocacy, and other needed services to clients in any environment including: the streets, shelters, jails, hospitals, homes, apartments, personal care homes, state hospitals, inpatient hospital, and rehab settings.
Collaboration with treatment team to support techniques that are consumer-first and included trust and respect
Majority (90%) of the work will be conducted in the community
Ability to develop treatment plans based on consumers stage of change for identified problems and needs
Ability to implement stage wise treatment interventions as well as exercise sound clinical judgment in routine, urgent, and emergent clinical situations
Link individuals to essential services and resources as clinically indicated
Requires extensive knowledge of Medicaid and Department of Behavioral Health and Developmental Services policies and regulatory guidelines.
MINIMUM QUALIFICATIONS:
BSN and 1 year of nursing preferred OR RN and 2 years of nursing. Valid Registered Nurse license. Medical experience a plus.
Valid Pennsylvania Registered Nurse license.
Valid driver license
Pre-employment Physical/TB/Hep B
Ability to drive an organization vehicle
Act 33/34/73 Clearances (Criminal/Child/FBI Department of Human Services)
Pre-Employment Drug Screen
Child Protective Services Law training (CPSL)
Working Knowledge and understanding of medication used in treating psychiatric disorders
