FlexibleJob Description/Essential Functions:Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice. Currently, we are seeking a Full Time Life Sharing Coordinator to work in our Developmental Services Division.This position will oversee and monitor Family Living/Life Sharing (LS) program.Listed below are some of the duties of the position:Participate in the interviewing and selecting of prospective families.Coordinate, write, and implement comprehensive assessments and service plans with the individual and family in accordance with the State Department of Public Welfare's 6500 regulations.Create and maintain a supportive/professional relationship with the families and individuals.Develop relationships with individuals interested in pursuing Life Sharing situations.Complete monthly visits and requires documentation.Travel in the community to assist with medical and psychological appointments.Job Qualifications:M.A. degree in Human Services/Social Sciences/Healthcare or a related field and at least 4 years of relevant experience in the Intellectual Disability field; B.A. Degree and at least 2 years of management experience; or any combination of experience, education or training that would provide the level of knowledge, skill and ability required.Knowledge and understanding of Family Dynamics.An ability to maintain complete confidentiality of client, family, and personnel concerns and records.Good analytical skills.Superior organizational and communication skills as well as the ability to keep clearly written records and notes.Prior experience in writing programming goals, and service outcomes.Ability to work independently and detailed oriented.Ability to travel to provider's homes and throughout Philadelphia.Horizon House offers competitive compensation and a comprehensive benefit package including: medical & dental, (9) nine paid holidays, generous paid time off, tuition reimbursement, retirement savings plan, voluntary tax sheltered annuity, transit benefit program, opportunities for advancement and more.
