Under the direction of GPS Nursing Leadership, the Global Patient Services (GPS) Clinical Nurse Coordinator supports all Global Patient Services initiatives. She/he provides coordination of case reviews for new patients, as well as provides culturally appropriate care and coordination of services for medically complex international patients who are receiving treatment at CHOP. The Clinical Nurse Coordinator is a liaison between the family, CHOP teams and external referring payor sources, establishing working relationships and ensuring an excellent patient family experience.Following established operational guidelines, the GPS Clinical Nurse Coordinator collaborates with CHOP teams to facilitate timely reviews of international patient referrals and develop comprehensive treatment plans based upon specialist recommendations, including arranging medical transportation when necessary. She/he works closely with families to address all questions and provide education about the treatment plan and anticipated length of stay. The GPS Clinical Nurse Coordinator works with GPS finance team to obtain financial clearance. She/he implements the treatment plan by coordinating recommendations, surgery, testing and appointments, communicating supportively with families and medical teams, and working with CHOP homecare and pharmacy. In addition, she/he writes comprehensive medical updates for each patient and requests approval for services to justify continued financial support for care at CHOP.The GPS Clinical Nurse Coordinator works closely with designated internal and external stakeholders to provide superior customer service and facilitate the coordination of excellent and timely care from the point of patient referral through safe repatriation to home country. In this role, she/he provides support to the ongoing operational and monitoring activities of the GPS program and ensures efficiencies of operational process. In doing so, he/she supports facilitation of new referrals to CHOP.The Global Patient Services Clinical Nurse Coordinator participates in all Global Medicine initiatives.What you will do

Supports all operations of Global Medicine.

Facilitates assigned new patient case reviews in an efficient and timely manner.

Ensures appropriate clinical services at CHOP are involved with each case.

Liaise with GPS Medical Director and CHOP providers to ensure treatment plan progression.

Efficiently coordinates appointments, surgeries and testing throughout care at CHOP.

Participates in interdisciplinary family meetings.

Presents concise patient summary in morning huddle for GPS clinic.

Collaborates with GPS Finance team to provide pertinent clinical information in order to develop cost estimates for patient financial clearance, and provide letters of medical necessity to obtain DME, home nursing, specialty medications and ambulance transport.

Serves as expert point of contact and liaison for patient/family, international providers, CHOP medical teams, and payor sources along the continuum of patient referral through care at CHOP and repatriation to home country.

Communicates effectively with payor sources to obtain patient financial support to continue care at CHOP and guarantee of payment for services.

Provides cultural education and serves a resource to CHOP staff when questions arise.

Maintains accurate data in GPS reporting systems and EPIC.

Maintains contact with referring physicians and payor sources to ensure excellent customer service, continuity of care upon repatriation, foster relationships and impact potential future referrals.

Bachelor's Degree Nursing RequiredExperience Qualifications

At least five (5) years of Intensive Care or Acute Care pediatric experience Required and

Previous experience in complex clinical settings that required coordination for medically complex patients. Required

coordination of services Preferred or

current EMR system, Salesforce, Excel, Sharefile, OneDrive PreferredSkills and Abilities

Broad pediatric clinical knowledge and coordination of services for medically complex patients

Critical analysis, problem-solving, excellent oral and communication skills.

Ability to understand and summarize clinical records, laboratory and radiographic data.

Ability to deal effectively and diplomatically with families, physicians and all levels of Hospital and Embassy personnel.

Ability to respond to families of diverse cultural and ethnic backgrounds.

Ability to function efficiently, independently, collaboratively and professionally.

High level of interpersonal skills with excellent customer service and responsiveness.Licenses and Certifications
