Employment Type:Full timeShift:Description:St. Mary Home Care and Hospice, a member of Trinity Health at Home is seeking compassionate Certified Nursing Assistants (CNA) looking to make a meaningful impact in their communities. We’re seeking motivated CNAs to serve patients in the greater Philadelphia 5-county area, bringing comfort and support directly to patients in their homes.C.N.A./Hospice/Home Care - Position Purpose:Our hospice/home health aides provide paraprofessional services to our patients in their homes, following physician orders and in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements and our policies and procedures.What You Will Do:
Follows care plan developed by RN.
Reports client changes in condition to RN, as described on care plan.
Assists clients with personal care activities including bathing, mouth care, denture care, skin care, grooming, bathroom privileges, transferring, ambulation, and dressing and undressing activities.
Assists clients with household tasks including, planning and preparing meals, feeding, light housekeeping, changing linens, making bed, and laundering clothes and bedding.
Measures and records client’s oral, rectal and axillary temperatures, pulse respiration and blood pressure as ordered.
Performs range of motion and other simple procedures as an extension of therapy services as assigned.
Assists clients in self-administration of medication. This procedure must be assigned by an RN and documented in the clinical record. The Home Health / Hospice Aide shall not administer medication of any kind
Minimum Qualifications:
High School diploma or GED required.
Active C.N.A. certification in PA
Completion of CNA course.
Completion of federally compliant training program as required by law/regulation and successful completion of a competency evaluation program.
Documentation of Three (3) year’s employment as an aide in an institution, home care, or hospice setting.
Must have current Driver’s license and reliable transportation to and from work site.
Current CPR training
Meets agency hiring criteria as defined in agency minimum standards (e.g. provision of professional references.)
Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with agency’s client and their family.
Must accept responsibility for maintaining skills, learning and adhering to agencies policies and procedures on an on-going basis
