It's fun to work in a company where people truly BELIEVE in what they're doing!We're committed to bringing passion and customer focus to the business.Summary:The Housing Support Case Manager is responsible for assisting 3 residents between the ages of 18 and 24 with a history of mental health and/or substance abuse to identify the skills they need to live independently. This is achieved by developing and facilitating individualized goal plans to promote independence, interdependence, and community integration. Staff work one on one with individuals both at the residence and in the community. The Housing Support Case Manager may also promote group activities and community experiences, as well as facilitate house and individualized team meetings. The Housing Support Case Manager reports directly to the Program Director.Full-Time (40 hours/week)- Monday-Friday, flexible.Wage Information:Starting wage: $20.00/hour, negotiable based on relevant experienceJob Description:

Coordinates and/or completes initial and ongoing functional assessments of individuals.

Coordinates and/or develops, reviews, and ensures implementation of individual recovery goals.

Assesses, evaluates, and documents individual progress and produces clear, accurate reports.

Develops and facilitates skills instruction in the residence and community.

Accompanies individuals on appointments related to their recovery goals.

Collaborates and cooperates with involved agencies, including Lebanon County Mental Health / Intellectual Disabilities / Early Intervention (MH/ID/EI) and Lebanon Housing Authority; actively participates in monthly team meetings and provides relevant information.

Knowledge of and utilize community resources and opportunities to promote community integration

Makes referrals to other social services, community agencies, and resources in order to meet individual needs; assists individuals in understanding and utilizing resources based on their recovery plan.

Willing to learn and model the five principles of trauma informed care; safety, choice, collaboration, trustworthiness, and empowerment.

Commitment to promote cultural, racial, language and gender access, diversity, equity and inclusion.

Ability to develop and implement behavioral, vocational, and future housing plans.

Ability to provide assessment and job development/support.

Ability to make sound, independent decisions and handle stressful situations.

Qualifications:This position requires one of the following combinations of education and experience:

Bachelor's degree in human services or related field from an accredited college or university; OR an Associates degree from an accredited college or university or 60 college credits and two years of working experience in programs for individuals with mental illness/ substance abuse issues/co-occurring disorders, or any equivalent combination of experience and training.

Additional requirements include:

Vocational and/or housing support experience preferred.

Must have a valid driver's license, good driving record, and access to a reliable vehicle to attend trainings, meetings, and/or transport individuals to appointments.
