Job DescriptionIndiana County Community Action Program, Inc.Title of Position: ESG Homeless Case ManagerImmediate Supervisor: Housing/Payee DirectorAreas of Responsibility:

Responsible for the identification of problems of homeless families and individuals.

Responsible for goal identification and service planning in conjunction with homeless families and individuals.

Responsible for service delivery, information, referral and monthly follow-up of homeless families and individuals and completing annual assessments.

Responsible for determining eligibility for the program, and work with HCM to help with housing search, housing habitability and lead based paint inspections.

Assuring the rental inspections pass, meet Fair Market Rent and Rent Reasonableness.

Specific Duties:

Determine program applicant's eligibility; and complete an in-depth needs assessment with each homeless family and/or individual.

Establish linkages with programs, and refer clients to services under ICCAP's umbrella as well as to services provided by outside agencies, follow-up to assure receipt of services.

Complete all necessary client information forms and obtain appropriate documentation for the files.

In conjunction with the client, identify major problem areas, develop a goal plan to impact those problems, and establish a time frame service plan to meet goals.

Help locate safe affordable housing that meets requirements of program.

In conjunction with HCM's, set up housing habitability and lead based paint inspections with ICOPD.

Meet with program participates and property owner/manager to have the ICCAP- landlord agreement and lease signed, and explain program to the landlord.

Complete a request for initial payments for rent, security and utility payments ensuring all required documentation is obtained before submitting request.

Maintain strict confidentiality. Get releases and work closely with the community contacts (case managers, agencies, landlords, utility companies, family members, etc.) to help program participates maintain stability.

Conduct weekly/as needed one-on-one interviews with program participants on the phone or in person to follow up on goals, referrals and other needs.

Visit the participants' residences at least once a month for on-site interview and housing inspections.

Re-evaluate program participates' eligibility and need of services every three months, and annually.

Complete rent determinations and submit to the Housing/Payee Director for authorization.

Work closely with the ICCAP's HAP Housing Case Managers, Housing Liaisons, Pathway Staff, Housing/Payee Director, and any other ICCAP staff person working with same client to ascertain coordination of services.

Provide services needed by case management clients that are not otherwise available, including, but not limited to, life skills education, budget counseling and transportation.

Maintain complete and accurate case files; entering intake information case notes, goal plans participants' progress, etc. through the agency client database, and HMIS in a timely fashion.

Scan required documents into ESG files on server.

Schedule annual inspections of ESG apartments.

Complete accurate records and reports as required.

Work as a team and maintain a positive attitude.

Other duties as assigned.

Screening Criteria:

Self-motivation to complete daily tasks and procedures without close supervision.

Ability to ensure the integrity of data entered.

Willingness to work with team members to achieve desired outcomes.

Knowledge of the respective county, employment is in Indiana, and familiarity with the social service network and community resources.

Demonstrated ability to communicate effectively with all members of the community.

Working knowledge of computers, including MS Word, Excel, an be willing to learn and utilize agency software programs such as ORS, as they pertain to this position.

Good organizational skills and have the ability to meet deadlines in a timely manner.

Must have a minimum of one year of case management experience and/or training in related field.

Must have a valid PA Driver's license, reliable transportation and be willing to travel in and out of Indiana County if required. Also must be willing to transport Clients using personal vehicle.

A Motor Vehicle Report and Act 33 & 34 clearances and FBI clearance with fingerprinting obtained upon hire.

Must have vehicle insurance to comply with agency standards of coverage.
