Assess and address clinical, social, psychological, and spiritual needs of patients within an episode of care.
Provide follow-up evaluations and continuity of care for ostomy and wound care patients post-discharge.
Utilize evidence-based practices and national guidelines (NPUAP, WOCN) to enhance patient quality, safety, and satisfaction.
Foster a compassionate, patient-focused experience through strong relationships with patients, families, and colleagues.
Serve as a mentor and educator by precepting students, new hires, and other healthcare professionals.
Conduct in-services, lectures, and training sessions to advance clinical knowledge and skills.
Support unit-based skin care committees and participate in product evaluations for quality and cost-effective care.
Act as a clinical resource, providing expert guidance on wound, ostomy, and skin integrity management.
Lead and engage in quality improvement initiatives to enhance patient care and workplace efficiency.
Promote a healthy work environment by fostering teamwork, cultural awareness, and professional development.
Apply the nursing process to create individualized, holistic care plans and effectively communicate them to care teams.
Participate in shared governance, interdisciplinary collaboration, and continuous professional learning.
Required:
Minimum of 2 years nursing experience
Registered Nurse (RN) Licensure, Certifications, and Clearances:
Current Pennsylvania licensure as a Registered Professional Nurse license or eligible for State Board Licensure.
Certification in Wound Care preferred
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
Special Skills and Abilities Required: Exemplary clinical judgment with critical thinking, analytical and problem solving abilities required
Ability to establish and maintain positive, caring relationships with executives, managers, physician, non-physician providers.
Ability to work productively and effectively within a complex environment and handle multiple and changing priorities.
Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state.
