We’re proud to serve a diverse and growing population and are seeking a passionate Family Nurse Practitioner (FNP) to join our dynamic care team.
Position Overview
We are looking for a compassionate, detail-oriented, and community-focused FNP who thrives in an independent practice setting. The ideal candidate will be committed to long-term relationships with patients, collaborate effectively with staff, and contribute to our mission of delivering high-quality, personalized primary care.
Key Responsibilities
● Provide comprehensive primary care to patients of all ages, from pediatrics to geriatrics
● Conduct well-child visits, annual exams, chronic disease management, and acute visits
● Order and interpret diagnostic tests, labs, and screenings
● Develop and implement individualized treatment plans
● Prescribe medications and manage medication therapies
● Collaborate with clinic staff to ensure seamless coordination of care
● Participate in quality improvement efforts and preventative health initiatives
● Maintain accurate and up-to-date patient records using our EHR system Qualifications
● Licensed as a Family Nurse Practitioner in the State of Oregon (or eligible)
● National Board Certification (AANP or ANCC) required
● DEA license or eligibility
● Excellent interpersonal and communication skills
