About FACT:

FACT (Forensic Assertive Community Treatment) is a transformative forensic mental health program, focusing on individuals with severe and persistent mental illnesses (SPMI) who have past or current justice involvement. Our multidisciplinary team provides community-based, client-centered, and unlimited services, improving mental health outcomes while addressing criminogenic risks to reduce recidivism. We offer individual therapy, crisis support, SUD counseling, supported employment, medication management, and much more.

Role Overview:

As a Counselor III in the FACT program, you’ll play a pivotal role in providing comprehensive support and therapeutic services for adults with severe mental illness and addiction issues. You'll work with individuals involved in the criminal justice system, helping them achieve their personal goals and reduce recidivism. Your expertise in trauma-informed care will be essential to understanding and addressing the effects of trauma on clients’ health and well-being.

Essential Responsibilities:

Coordinate and monitor community-based case management services for FACT clients.

Advocate for clients, helping them access essential services such as housing, employment, and legal support.

Provide direct social, emotional, and instrumental support to clients.

Teach and assist clients with daily living skills, from grocery shopping to employment opportunities.

Conduct mental health assessments and develop individualized treatment plans.

Implement service plans, focusing on minimizing illness and maximizing strengths.

Deliver evidence-based interventions to help clients reduce criminal recidivism.

Handle crisis situations, ensuring a trauma-informed environment for all.

Maintain up-to-date documentation, ensuring compliance with regulations.

Collaborate with other service providers and community justice departments.

Qualifications:

Education: Master’s Degree in a behavioral science field from an accredited institution. Must meet Oregon's QMHP criteria.

Experience: Non-profit experience preferred. CADC credential is advantageous. Forensic background also helpful

Skills:

Conduct mental status assessments and diagnose per DSM.

Familiarity with Motivational Interviewing, trauma-informed care, and Illness Management and Recovery.

Strong knowledge of criminogenic risks and needs, and evidence-based practices.

Computer literate.

Other:

Valid driver's license and ability to pass DMV check. Requires use of personal vehicle or access to reliable transportation.

CPR/First Aid certification.
