Are you ready to take on a dynamic role that makes a difference in the lives of patients in Portland, Oregon? We're seeking a motivated and detail-oriented individual to join our team as a Panel Coordinator, Primary Care, at Cascadia Health.
This position will work at one or more of our four outpatient health centers in the Portland Metro area. The schedule is typically Monday through Friday, 8:30 a.m. to 5:00 p.m.
Job Overview:
As a Panel Coordinator, you'll be at the heart of our patient-centered care approach, supporting the implementation of our innovative Patient Centered Medical Home model. You'll play a crucial role in coordinating services, analyzing patient data, and ensuring optimal patient involvement and team performance.
Key Responsibilities:
Evaluate patient records and coordinate services for assigned panel, focusing on preventive care and identifying gaps in care.
Communicate directly with patients to discuss preventative care needs and refer them to appropriate clinical staff.
Track progress on key clinical quality metrics and assist in developing standards for chronic disease management.
Collaborate with Medical Home Specialists to support the ongoing development of our Primary Care program.
Maintain patient charts, schedule appointments, and ensure compliance with insurance requirements.
Provide patient education and support to promote health plan issues and preventive care.
Attend staff meetings, participate in agency committees, and complete mandatory education requirements.
Qualifications:
Licenses, Certifications and/or Registrations:
Current Healthcare provider BLS certification.
Certified Medical Assistant (CMA) from American Association of Medical Assistants (AAMA) or equivalent certification
Experience/Knowledge:
Minimum of 1 year of experience in a medical office setting, including direct patient contact and scheduling appointments.
Knowledge of medical terminology, CPT, and ICD-10 CM coding methodology.
Excellent communication and interpersonal skills, with a strong customer service orientation.
Proficiency in MS Office applications and electronic health record systems.
Bilingual in Spanish and English preferred.
