Department: Cow Creek Behavioral Health

Location: Roseburg, OR

Minimum Salary: $90,000

POSITION PURPOSE:

The Behavioral Health Consultant is responsible for direct patient care by providing screening, brief intervention, brief treatment and referral to patients in the Primary Care Clinic. The BHC will consult with the Primary Care Providers and integrated care team members and will provide support to the patients in Primary Care.

ESSENTIAL FUNCTIONS:

Provides onsite, telehealth and community behavioral health services in primary care clinic.

Conduct comprehensive assessments of patient mental health needs with patients as referred by primary care providers.

Develop individualized treatment plans in collaboration with patient.

Provide counseling and therapy sessions to patient, utilizing evidence-based practices.

Directs intervention toward helping patients develop self-management skills.

Monitor patient progress and adjust treatment plans as necessary.

Collaborate with other healthcare professionals to coordinate patient care

Educate patient and their families on mental health issues and coping strategies.

Provides crisis intervention and evaluation services as needed.

Maintain accurate and up-to-date patient records.

QUALIFICATIONS:

Master's degree in behavioral health, social work, or counseling

Current licensure with the Oregon Board of Psychologist, or any other of the following licensure: LCSW, LPC, or LMFT.

Minimum of 2 years of experience working in behavioral health counseling

Strong knowledge of evidence-based counseling techniques

Excellent interpersonal and communication skills

Demonstrated ability to work effectively with diverse populations

Strong organizational and time management skills

Ability to maintain confidentiality and adhere to ethical guidelines
