A person functioning in this capacity will facilitate services and supports to assist eligible individuals and families to identify and achieve personal goals.
Under the general supervision of the Support Services Manager, the Service Consultant assists the individual and their families/guardians to explore and gain services and supports from a variety of sources. Assures that services delivered adhere to all regulatory requirements, as well as UCBDD policy, regulations and procedures. The Service Consultant must strive to meet the UCBDD published Mission Statement and exemplify the UCBDD defined Core Values.QUALIFICATIONSEducation/Experience Bachelor's Degree in Human Services, Business Administration, or related field.Credential/License Must possess or meet all of the ODODD requirements to possess and maintainSupport and Service Administrator Certification under OAC 5123:2-5-02.Must obtain and maintain First Aid and CPR certification.[]{style="col"}
