Provides care focused on the spiritual, physical, social and emotional needs identified. Assesses, plans, implements and evaluates the patient’s physical, psychological, and socio-cultural needs. Collaborate with other members of patient care team to achieve goals. The RN Case Manager is committed to the goals, philosophy, and objectives of New Life Hospice and CHP.Minimum Knowledge, Skills and Abilities1.Professional knowledge of nursing theory and practice at a level normally acquired through completion of an Associate’s degree in Nursing, Diploma in Nursing, and one year’s current experience within the last five years in an acute care setting is necessary.2.Approximately one to two years experience necessary in order to become fully skilled in assessing patient needs, implementing and evaluation effective plans of care and being knowledgeable with hospital and departmental policies and procedures.

Current license as a Registered Nurse in the state of Ohio.

Maintains annual certification in CPR.

Must possess self-direction, motivation, and time management skills. Must be able to develop and maintain effective working relationships with people at varying ages, education, social, economic, and ethnic backgrounds. Flexibility to adapt and respond to a rapidly changing environment.Working ConditionsNormal patient care environment with little exposure to noise, dust, temperature and the like. Very frequent exposure to moderately adverse working conditions due to performance of certain distasteful patient care activities. Ability to concentrate and pay attention to detail fifty percent or more of work time.
