What you need to know about this position:
The Outpatient Medical Social Worker provides social work services including assessment, intervention, education and disposition planning to meet the psychosocial needs of patients of any age, and/or their families.
The Social Services Specialist will identify and provide therapeutic interventions necessary to meet their holistic needs.
The Social Services Specialist will communicate with patients, families, and staff to ensure development of a safe and appropriate continuing care plan.
The role assists patients and their families in resolving personal and environmental issues that would negatively impact their plan of care, predispose an individual to illness, or interfere with obtaining maximum benefit from medical care.
The base hourly rate for this position is $27.93. Individual compensation is determined for this position through years of directly relevant experience. The hourly compensation is only a portion of the total rewards package, and a comprehensive benefits program is available for qualifying positions.
This full-time position will require you to work, 8:00AM-4:30PM. Monday through Friday.
What is required for this position:
Education and/or Experience
Master's degree in Social Work or related Mental Health field from an acredited university required.
Certifications, Licenses, Registrations
Indiana Licensed Clinical Social Worker LCSW required.
Basic Life Support (BLS) certification required.
