Results to be achieved by RN Case Manager:
I will provide the Staff LPN with a current client visit schedule monthly.
I ensure that all client home visits and supervisory assessments are completed on time.
I review and approve all LPN client assessments.
I review and approve all LPN performed Supervisory Assessments.
I perform a monthly client chart audit to ensure that all client charts meet agency standards.
I perform new client case assessments and client well visits as needed.
I supervise all field nursing staff.
I document any unusual client findings and report changes in condition to family, physician, and other individuals accountable to the client as necessary.
I participate in the Home Health Aide Certification Classes and In-Service Classes.
I review field staff visit slips and complete periodic Visit Slip Evaluation Forms.
I document return demonstration skills for all certification class participants on their individual training log.
Assist the Director with staff disciplinary issues and development of policy and procedure.
Standards for RN Case Manager :
Must be punctual.
Appearance must be as indicated in the Employee Handbook.
Demonstrate good moral and ethical business practices
Must pass State Background Check
Knowledge, Skills and Abilities Needed for RN Case Manager :
Must possess a valid and current OHIO RN license in good standing
Must possess good nursing assessment skills
Ability to follow the instructions of the Director and uphold the agency’s policies
Experience Needed for RN Case Manager :
At least 1 year nursing experience (preferably with elderly and disabled)
Documented supervisory experience in the nursing field
