Results to be achieved by Home Health Aide/Caregiver -“I know I’m doing my job when…”
I provide all direct client care including baths as ordered according to the care plan.
I provide personal care and homemaker services for my clients per the schedule provided by the office (Scheduler) in a consistent and timely manner.
I stay within the scope of my position by not performing a service for which I am unqualified.
I immediately report all changes in the client’s condition and needs, client injuries and incidents to either the Director/Administration/RN.
I turn in all required EVV documentation on time, completed and accurate, as specified in the Policy and Procedure Manual
I adhere to the agency dress code by always being in uniform, including my picture ID.
I follow the HIPAA policy and procedures used to protect the privacy of each client.
I am an advocate for every client by honoring his/her dignity.
Ambulates and exercises the patient as assigned.
Performs simple procedures as an extension of the therapy services, e.g., range of motion (ROM) exercises as assigned per the RN or PT.
Assists with medications that are ordinarily self-administered as assigned.
Attends in-service and continuing education programs as scheduled and necessary and completes 8 CEU’s each year.
Attends patient care conferences as scheduled.

Demonstration of a kind and caring attitude

Demonstration of moral and ethical business practices
Appearance as indicated in the Policy and Procedure Manual
Ability to carry out directions, read and write.
Maturity and ability to deal effectively with the demands of the job.
Understanding and adherence to Agency policies and procedures.
Experience needed for Home Health Aide/Caregiver :
Must meet OH state standards for Home Health Aide training program and competency
1 year verification of supervised hands-on personal care experience in last two years and successful completion of a competency evaluation program OR
Valid STNA or valid completion of training program that meets the state requirements without continuous lapse of employment for 24 months

Works at patient homes and travels to/from patient homes.
