Assesses patient and family's psychosocial risk factors through evaluation of prior functioning levels, appropriateness and adequacy of support systems, reaction to illness and ability to cope.

Assesses patient and family's financial, social, emotional, educational and environmental resources and identifies needs based on patient's current medical condition.

Develops an individualized plan of care based on the findings of the assessment and needs of the patient and family/caregiver as related to the patients medical condition.

Participates in Discharge planning activities for complex patients in order to ensure a timely discharge and to provide appropriate linkage with post-discharge care providers.

Other duties as assigned.

Education:

Bachelor's Degree in Social Work from an accrediated school of Social Work.

Possesses knowledge of and practices in accordance with the NASW Code of Ethics.

Certifications:

Licensed Social Worker (LSW) in the current State.Social Worker or Case Management Certification (CCM) preferred within one year of eligibility.Proof of Basic Life Support (BLS) through American Heart Association (AHA) must be presented upon hire.

Work Experience:

Minimum of 3 years' experience in healthcare system to include exposure to and experience in pre acute and post-acute care preferred.Hospital Social Work experience preferredKnowledge of community resources preferred.Excellent communication and negotiation skills necessary to work collaboratively with multidisciplinary teams for optimal outcomes.

Demonstrated ability to work independently as well as with teams.

Must be flexible, open-minded and adaptable to change.

Physical Requirements:

Requires normal or corrected hearing and vision to normal range.

Ability to perform work in a stationary position for extended periods.

Ability to communicate and exchange accurate information

Ability to work with physical records, such as retrieving and filing. Ability to operate a computer and other office equipment.

Ability to travel throughout the hospital system.

Requires normal or corrected hearing and vision to normal range.

In some locations ability to lift up to 10 pounds
