Patient Care Coordinator

Patient Care Coordinator- a team member to help patients coordinate appointments with their schedules and the clinic hours. Coordinators would also help patients coordinate appointments with referring specialist clinics. A PCC would help work with clinic team members to help the clinic answer phones, make appointments, coordinate treatment. They would also be the patients' way to coordinate communication between providers and patients.
