The Nurse Practitioner (NP) or Physician Assistant (PA) provides patient care services, including the assessment and management of patients in an outpatient environment while following established standards and practices as a Primary Care Provider (PCP). The NP/PA PCP is responsible for managing care for a panel of patients and works with an interdisciplinary team of clinical staff to establish medical protocols and treatment regimen. The NP/PA PCP collaborates with the physician in providing exceptional patient care with optimal outcomes. The NP/PA PCP provides an exceptional patient care experience through enhanced patient/physician communication. This individual ensures continuity of patient care in collaboration with other health care professionals and may assume additional clinical responsibilities within the department as needed."Job Description:Essential Duties & Responsibilities including but not limited to:This individual continuously works in direct communication with patients, physicians, staff and other professionals to coordinate patient care. The incumbent, with the collaboration of the physician, performs the following duties: 1. Performs thorough physical examinations while obtaining complete medical history to document findings. 2. Maintains patient record and relevant statistics by documenting patient information and care provided. 3. Evaluates/interprets data in an effort to ascertain preliminary diagnoses and to make therapeutic plan recommendations. 4. Responsible for the longitudinal care of the patient and the adherence to screening and preventive guidelines. 5. Administers medication and injections in compliance with state law and employer guidelines. 6. Provides instruction to patient, family and care providers relative to required medications and treatment plan. 7. Acts as a care advocate in providing recommendations (community, health plan, etc.) for health and wellness solutions. 8. Obtains specimens for laboratory analysis, schedules and prepares patients for diagnostics testing. Prepares the patient and facility for procedures. 9. Provides patient consultative services through telephone triage services. Documents all necessary information, provides instructions and materials as required. Provides patient care based on the documented consistent assessment of each patient's unique needs. 10. Participates in activities that contribute to continuing education and research (ie precepting students). 11. Prescribes appropriate medications 12. Collaborates with Behavioral Health Team when needed to facilitate optimal patient care. 13. Call coverage and responsibility to be determined by the Department Chair. 14. Refers to other services as necessary to provide appropriate patient care.Organizational Requirements:Maintain strict adherence to the Lahey Hospital and Medical Center Confidentiality policy. Perform all other duties as needed or directed to meet the needs of the department.Minimum Qualifications:Education: Nurse Practitioner: Current state RN licensure in MA, confirmation of Board Certification as a nurse practitioner by a recognized national certifying body that has established standards for nurse practitioners (ANCC, AANP). BSN or MSN with concentration in specialty as nurse practitioner preferred. Successful completion of an approved Nurse Practitioner program required. Professional nursing experience and/or nurse practitioner experience with prior clinical experience preferred. Physician Assistant: Current certification and graduate of an accredited Physician Assistant Program. Documentation of registration for next NCCPA Board administration or successful completion of NCCPA Board and maintenance of appropriate CMELicensure, Certification & Registration: Current license and certification to practice in the Commonwealth of Massachusetts. DEA number and Mass Controlled Substances registration number. Must be able to meet credentialing requirements. Basic Life Support (BLS) certification.Experience: Minimum of 3 years of experience as a NP or PA with solid foundation in Primary Care.Skills, Knowledge & Abilities: Good communication skills. Ability to interact effectively with medical staff, nursing staff, and other clinical and administrative colleagues as well as with patients and their families. Knowledge of clinical procedures and protocols. Demonstrated ability to maintain confidentiality and ability to provide appropriate emotional support.Key Relationships:1 Department Chair Reporting relationship2 Physicians Seeks out for guidance and direction with patient care3 Administrative Director To identify and discuss any clinical and operational issues that may arise on an ongoing basis.4 Medical Records Requests patient medical records as needed.5 Nurses To provide direction with patient care.6 Patients and Family Members Diagnoses and treats patients.
