KEY RESPONSIBILITIES:· Responsible for taking and processing Palliative Care referrals.· Provide support to the Palliative Care Nurse Practitioner and Medical Director.· Will schedule visits to support goals of care as ordered by MD.· Coordinate and communicate patient care goals between all providers.· Make calls to present the palliative program, obtain legal consents, and assess interdisciplinary needs.· Ensure communication of assessment and reassessment findings to attending physician.· Complete all documentation accurately, timely, and in accordance with company policy and procedure.· Inform palliative team of all new admissions and discharges.· Coordinate referrals utilizing the referral process.· Serve as a liaison for medical directors and nurse practitioners, enhancing effectiveness and compliance.· Run billing report and address validation holds/ billing errorsTo apply please email [email protected], CERTIFICATION, AND EDUCATIONAL REQUIREMENTS:· Current, active, and unrestricted Registered Nurse (RN) OR Licensed Practical Nurse (LPN) licensure in the state of practice· A minimum of three (3) years of nursing experience· Palliative care, hospice, or home health care experience is preferred· CHPN (Certified Hospice and Palliative Nurse) is preferredFamily Makes Us Stronger.
